Our Silver Star program is designed to promote independence and autonomy for participants who might otherwise require nursing home care. We provide high-quality healthcare and personalized support services that enhance quality of life, while also supporting caregivers through training, support groups, and respite opportunities. Our goal is to deliver convenience, peace of mind, and compassionate care for both participants and their families. With approval from the Interdisciplinary Team*, participants gain access to a range of valuable benefits.

Regular Access to our Silver Star Day Health and Wellness Center including*
- A fully-staffed Medical Clinic
- Physical, Occupational, Speech and other needed Therapy sessions
- Social Service Referrals
- Outdoor Patio and Gardening Area
- Exercise Equipment
- Social and Recreational Activities
- Hot lunch and Snacks
- Transportation to and from the Center
*Prior authorization by the Interdisciplinary Team is required for services (with the exception of emergency services).
*PACE Participants may be held financially responsible for the cost of unauthorized services or services provided outside of the PACE agreement.
